Home Health Aide (HHA)

The ideal candidate will excel at providing physical assistance with common tasks like light housekeeping and meal preparation, as well as communicating with family members.
Our organization is looking for a qualified Home Health Aide who can work independently and effectively communicate with patients.
Responsibilities:
Perform physical care for patients including housekeeping chores, transferring, bathing, dressing, and feeding patients.
Be emotionally supportive to patients.
Communicate effectively with family; provide feedback to help improve home care.
Communicate with other caregivers to coordinate patient care.
Encourage patients to be physically active.
Ensure that patients adhere to a prescribed treatment plan and schedule.
Assist with ADLs as appropriate.
